What We’re Looking For

Trillium Health Resources has a career opening for a Director of Integrated Care Implementation (DICI) to join our Clinical Services Leadership team. This position has primary responsibility for the strategic implementation of integrated care models across six Clinical Service lines, directly supporting the respective Assistant Vice Presidents (AVPs) and Vice Presidents (VPs). The ideal candidate demands a highly strategic, collaborative, and results-oriented leader with an understanding of integrated care principles, managed care operations, and the complexities of large-scale system transformation. On a typical day, you might:

  • Execute on directives, deliverables, and requests in a timely manner.
  • Establish and execute key performance indicators and operational metrics to determine measures of success, quality outcomes, compliance, and clinical effectiveness.
  • Develop and articulate a comprehensive strategy for integrated care implementation across the designated service lines, aligning with the organization's overall strategic goals and the requirements of the Tailored Plan.
  • Serve as a strategic partner and master facilitator.
  • Oversee and direct complex, cross-functional projects related to integrated care implementation, ensuring adherence to timelines, resource allocations, and quality standards.
  • Lead and facilitate Clinical Services change initiatives necessary for successful integrated care adoption, including stakeholder engagement, communication strategies, training programs, and the development of a change-ready culture.

Qualifications: 

Education/Experience:

A Bachelor’s degree with three (3) years of combined experience in healthcare or business with a focus on strategy, operations, project management/analysis, program management, or organizational development.

 

Preferred Education: 
Master’s degree preferred; Healthcare Administration, Public Health, Social Work, Business Administration, IT, Human Services, or a related field.

Preferred Experience:

  • Supervision of two (2) or more employees preferred
  • Experience in leading and managing complex, moderate to large-scale implementation projects within a healthcare or business setting
  • Knowledge of integrated care models, principles, and best practices
  • Understanding of change management methodologies and the ability to drive organizational transformation

 

License/Certification:

Must have a valid driver’s license

 

Preferred License/Certification:

Certificates or industry certifications in Project Management, Program Development, Change Management, Business, Strategy, Data Analytics, or Leadership.

 

Location: Must be able to travel to a Trillium office locations and within catchment as required.
