Treatment, Schedules and Billing. Also, supervises qualified student interns as the clinical and high
school level.

ANNUAL SALARY: $80,000.00 - $95,000.00 Annual Salary- With Benefits & Paid Time Off
REPORTS TO: Chief Clinical Officer
ADDITIONAL POSSITIONS and OPTIONS MAY BE AVAILABLE: Part-time hourly positions may be
available at certain locations for Non-Exempt (Regular Full-Time) and Non-Exempt (Regular Part-Time)
employees. Benefit contracts may be available for the Regular Full-time Hourly who is willing to work
34- minimal hours and manage a patient load of evaluations and treatments during that time period.
Regular Full-Time will be held to a reasonable productive standard.
NON-EXEMPT HOURLY PAY RATE RANGE: $41 to $47.50 per-hour

Core Health Partners (CHP) is seeking a full-time Occupational Therapist to fulfill the following job duties:
1. Evaluate patient needs and create treatment plans 2. Provide and oversee physical/aquatic therapy
services 3. Direct and instruct patients with chronic disease conditions, physical disabilities, or injuries 4.
Supervise a team of: - Certified Occupational Therapy Assistants (COTAs) - Aides - Student interns
Work Environment: - Integrated team setting - Clinic-to-community model of therapy. Must be able to
adapt, innovate and operate in a non-typical clinical environment.
Job Responsibilities:
Evaluation and Assessment: 1. Evaluate patient communication and swallowing skills 2. Assess cognitive-
communication, speech, language, and hearing abilities 3. Identify patient needs and goals Treatment
Planning: 1. Develop individualized treatment plans 2. Set realistic goals and objectives 3. Prioritize
treatment strategies Patient Care: Pediatric: 1. Assess and treat speech, language, and feeding disorders
in children 2. Develop strategies for parents and caregivers to support communication development 3.
Collaborate with schools and healthcare teams Adult: 1. Assess and treat speech, language, cognitive-
communication, and swallowing disorders 2. Develop strategies for communication and swallowing
rehabilitation 3. Collaborate with healthcare teams and families.
II. Patient Population - Work with patients across the lifespan (adult and pediatric), however we may be
able to customize preference and skill set depending on patient load and referrals.
III. Service Delivery - Our approach diverges from the conventional sick-based or clinical model, as we
deliver services in innovative community settings where families reside, learn, and thrive. This distinctive
methodology demands creativity, adaptability, and collaboration to effectively address the needs of our
patient-centered care pathway.
IV. Professional Relationships – Must be able to develop strong relationships with a network of referring
physicians
V. Data Management and Reporting - Collect and report data to support patient care and outcomes -
Develop instructions and reports to assist caregivers and community non-profits in continuing patient
care beyond medical billing
VI. Collaborative Care - Contribute to maintaining patient wellness through collaborative efforts
VII. Staff Support - Serve as a staff point of contact for general assistance and emergency situations, hold
team meetings with minutes and use technology to keep staff informed.
VIII. Other Duties as Assigned - As a collaborative team, all staff members are expected to embody the
company's mission and integrate our innovative care model into the community setting. Additionally,
staff may be assigned other duties that support the company's mission and contribute to its success.

MINIMUM ENTRY REQUIREMENTS: - Active, unrestricted license to practice as a Speech Language
Pathologist in the state of Florida - American Red Cross certifications in: - CPR-/AED/First Aid - Blood
Borne Pathogens Training - Proficient computer skills, including: - Microsoft Office (Word, PowerPoint,
Excel) – Ability to be proficient in navigating our Electronic Health Record (EHR) documentation system
of software.

Rooted from a group of visionary YMCA professionals from Des Moines, Iowa Milestones

The expected hourly commitment for salaried employees is: - Minimum of 40 hours per week onsite -
Exempt from hourly measurement, meaning they are not clocking in and out or tracked by hourly
productivity - Expected to work towards individual and department goals, as well as the organization's
mission - Salary is not tied to hourly performance, but rather to overall performance and achievement of
goals In essence, salaried employees are expected to be fully engaged and committed to their roles,
working a standard full-time schedule (40 hours/week) onsite, without being tied to a specific hourly
quota. Their performance will be evaluated on their progress towards goals and overall contribution to
the organization, rather than hourly productivity.

obtain prior manager approval before engaging in outside employment to avoid conflicts of interest or

Medical Specialty (CHP offers a variety):
1. Pediatrics: Working with children from birth to adolescence, addressing speech, language, and feeding
disorders.
2. Neurogenic: Focusing on adults and children with neurological disorders, such as stroke, traumatic
brain injury, dementia, or Parkinson’s.
3. Fluency Disorders: Specializing in stuttering, cluttering, and other fluency disorders.
4. Voice Disorders: Addressing voice problems, such as hoarseness, vocal nodules, or paralysis.
5. Dysphagia: Focusing on swallowing disorders, working with patients to improve safe eating and
drinking practices.
6. Bilingual or Multilingual Services: Providing assessment and treatment services in languages other
than English.
7. Autism Spectrum Disorder (ASD): Working with individuals with autism, addressing communication,
social interaction, and behavioral needs.
8. Gerontology: Specializing in the communication and swallowing needs of older adults.

Schedule
• Exempt /Flexible with 40 hours minimal is expected onsite
• Flex Day shift-or Evening shift- Productivity Based
• Monday to Friday (Flex weekday to weekends and Flex Hours is possible)
• Weekends as needed/ if needed
Ability to Relocate:
• Collier County, FL (or SW Florida Area): Relocate before starting work is required.
