Not Forgotten Home & Community Services provides direct care support to individuals with Intellectual & Developmental Disabilities. We have a need for a full-time Registered Nurse to support the Residential, Day Program, and Habilitation Services Departments with the following activities:

Duties

  • Support individual medical needs that occur and require RN follow up: ie. medication orders, medication reviews, staff coaching.
  • Contact/Liaison for individual's medical concerns with healthcare providers, families and team members
  • Maintain familiarity with the current status and medical needs of all individuals in the program, their medical history, and identify high risk individuals and ensure specialty care needs are met
  • On-site evaluation of individuals and the completion of nursing assessments
  • Assist in the medication procurement for individuals as well as the clinical and skill development and training for all staff

Qualifications

  • Current RN license in good standing.
  • Proven nursing experience in home & community-based services focusing on IDD community
  • Two years of nursing experience, including dealing with emergencies and/or acute medical conditions
  • Familiarity with PA Office of Developmental Program requirements and 6400 regulations.

Schedule is a set rotation at 40 hours per week. In the event of a medical emergency, nurse must be able to answer after hour/weekend phone calls.
