We are currently seeking compassionate and experienced Physical Therapists (PTs) and Occupational Therapists (OTs) to join our growing home health team serving patients throughout Santa Barbara County and West Ventura County Areas.

In this role, you will provide in-home therapy services, develop individualized treatment plans, and work closely with patients, caregivers, physicians, and our interdisciplinary team to ensure the highest quality of care.

Responsibilities

Physical Therapists (PT)

• Travel to patient homes throughout Santa Barbara and West Ventura areas.
• Perform initial and ongoing physical therapy evaluations.
• Develop and implement individualized Plans of Treatment.
• Assess patient mobility, strength, balance, and functional abilities.
• Monitor progress and modify treatment plans as needed.
• Educate patients and caregivers on exercises and recovery strategies.
• Maintain accurate and timely documentation.
• Coordinate care with physicians and healthcare team members.

Occupational Therapists (OT)

• Travel to patient homes throughout Santa Barbara and West Ventura areas.
• Perform initial and ongoing occupational therapy evaluations.
• Develop individualized treatment plans focused on improving daily living skills and independence.
• Assess patients' ability to perform activities of daily living (ADLs).
• Recommend adaptive equipment and home safety modifications as needed.
• Educate patients and caregivers on therapeutic techniques and strategies.
• Maintain accurate and timely documentation.
• Collaborate with physicians and healthcare team members to support patient goals.

Qualifications

• Current and valid California Physical Therapy License (PT) or Occupational Therapy License (OT)
• Minimum of 1 year of clinical experience preferred
• Valid Driver's License
• Reliable transportation
• Ability to travel within assigned service areas (up to a 30-mile radius)
